Here Are The Requirements For Obtaining A Passport

Here are the requirements to obtain a passport form the Embassy of the Republic of Haiti in Washington DC

You need the Expired Haitian passport

You need your original Haitian birth certificate or a copy certified with the original or a birth certificate from the Haitian National Archives

You need two different forms of identitification

You will also need Two (2) color pictures (passport size) no more than six (6) months, bearing the seal of the photographer and date. The bearer will register his/ her name on the back of the pictures

If you are married you will need the original Marriage certificate if you are is a married woman applying for a Haitian passport.

If you are a divorced Haitian woman, you will need Act of Divorce.

If you are a Haitian woman and your husband is dead, you will need the Death certificate of your husband.

If you are a minor applying for a Haitian passport than you will need a parental authorization signed by one of your parents or legal guardian.

What if you loss yourold passport and brithcerificate and you need to renew or get a new one?I had ID from the US but I can't find the old passport and brithcerificate?I also have my visa with all my information on it. I need this do before Aug. of this year. please write back.

u need to have that birth certificate because without it there's nothing they can do for you. That's how it is in New York.

Hi,

I recently went to the Hatian Consulate New York office, with all the requirements: Old Passport, New Passport Pictures, 2 Proper ID's, and a COPY of my Birth Certificate (in english and Creole on the same page).

But I was denied (by the way, the lady was rude as hell)and I really need this Passport to start traveling, I have a lot of plans coming up....

Please, can you tell me how to get this done as soon as possible....
